What you'll do

• Assist with bathing, toileting, dressing, grooming, hygiene, skin and nail care, eating, and nutrition
• assist with ambulation, transfers, and range-of-motion exercises
• check vital signs, measure intake and output, monitor blood glucose, and document changes
• follow patient care plans with home-health nurses and aides
• notify a supervisor of issues requiring changes in care, medication, or equipment.
