What you'll do
- Develops and implements discharge planning within an interdisciplinary healthcare team.
- Identifies and removes barriers that prevent optimum access to needed post-acute care.
- Works collaboratively with providers to identify discharge barriers and delays.
- Optimizes efficient use of acute care hospital days and prevents prolonged length of stay.
- Provides clinical leadership to healthcare teams.
- Ensures daily goals are met according to the patient’s plan of care.
What we're looking for
- Education
- Graduate from an accredited school of nursing
- Experience
- Previous experience in acute care healthcare setting.
