What you'll do

• Provide consultation and counseling to people with intellectual and developmental disabilities receiving Medicaid Waiver services
• conduct assessments and develop individualized multidisciplinary treatment plans
• consult with and train family members and host-home providers
• collaborate with interdisciplinary team members
• meet monthly billed-unit productivity goals
• participate in regional leadership meetings and help address health, operational, and event-planning matters.
