What you'll do

• Conduct client intakes and assessments
• hold in-home case management meetings
• develop care plans and coordinate services
• respond to intake callers and follow up
• review and authorize services monthly
• provide crisis intervention and outreach
• prepare funder reports and case records
• offer short-term or ongoing psychotherapeutic support as needed
• develop informal support systems
• refer clients for specialist evaluations
• select, negotiate, and budget supportive services.
