What you'll do

• Direct and manage day-to-day Social Service department operations in accordance with regulations and community policy. Supervise department operations, assign work and priorities, set goals, orient and educate team members, build relationships with residents, families, and regulatory agencies, assist with the department budget, and oversee policy compliance. Provide psychosocial interventions
• conduct resident assessments
• create and update care plans and progress notes
• coordinate outside services
• attend resident council meetings
• support families coping with placement, illness, disability, and grief
• assist with discharge planning
• and educate staff on psychosocial issues and patient rights.
