Case Manager Supervisor

The supervisor develops and supports behavioral health case managers (BHCMs) so support coordination is delivered consistently and in a person-centered way. Through coaching, case review, performance assessment, and field observation, the role helps maintain service quality and address participant needs.

What you'll do

• Train BHCMs on documentation and service delivery expectations
• manage caseload assignments
• hold regular individual case-review sessions
• evaluate staff performance
• review support coordination through meetings, reports, training, and observation
• accompany staff on random home and community visits
• identify and arrange training
• teach and monitor person-centered practices
• review timesheets and mileage reports
• participate in management meetings.

Why this role

Requires a bachelor’s or master’s degree in social work or a related human services field, eight years of paid post-degree support coordination experience, including two years of supervisory experience, and reliable transportation. Day travel to outlying parishes is required as needed.
