The Family Physician, Manhattan role is onsite in New York, NY, full-time, with pay of $200,000–$205,000 per year. Care For the Homeless provides care to the center’s patients, including people experiencing homelessness and other underserved populations. The physician provides patient-centered primary care, develops treatment plans, coordinates referrals, and documents care. The role contributes to the PCMH team through care coordination and consultation with other healthcare providers and staff, and participates in the on-call and emergency coverage schedule as directed by the CMO.

What you'll do

• Perform history and physical assessments
• provide care consistent with accepted medical practice
• develop treatment plans that include education and follow-up
• make referrals as appropriate
• communicate and document care with other providers
• maintain complete medical records
• supervise or consult with nurse practitioners and physician assistants as requested
• participate in on-call and emergency coverage
• coordinate care with medical case management and health education.
