Transition Care Coordinator

What you'll do

• Orient and educate staff, patients, and families.
• Maintain knowledge of assistance programs and community resources.
• Monitor and evaluate the plan of care.
• Foster peer support and community engagement.
• Collaborate with healthcare providers.
• Participate in QI projects and case conferences.
• Educate and advocate for patients.
• Coordinate counseling.
• Provide documentation.
• Participate in crisis assessment.
• Practice effective advocacy.
