What you'll do

• Participate in developing and updating person-centered plans
• support service recipients and families in treatment participation and progress monitoring
• coordinate activities under the plan
• assist with case management, referrals, scheduling, and community-resource linkage
• support discharge planning and transitions to community, home, or family
• document encounters and incidents
• maintain confidential records
• complete intake packets and required training
• participate in the first-responder on-call system.
