Hospice Community Specialist

What you'll do

• Develop referral relationships with physicians, hospitals, senior living communities, rehabilitation centers, and skilled nursing facilities
• identify opportunities to grow admissions
• build partnerships with physicians, case managers, social workers, and healthcare leaders
• support patients and families with empathy and clear information
• develop and execute a territory plan
• evaluate and adapt outreach strategies
• represent the organization’s values.
