What you'll do

• Identify patients appropriate for RN case management
• collaborate with primary care providers, nurses, pharmacists, patients, and families on care plans and self-management goals
• coordinate with departments and community agencies
• arrange follow-up, laboratory, and other visits and follow up on missed appointments
• use learning assessments, teaching guidelines, and electronic records to document interventions.
