What you'll do

• Visit nursing facilities and work with PASRR clients toward goals
• provide personal assistance for doctor visits, community retailers, and events according to client needs
• enter data into the system
• attend training and plan-of-care meetings and reviews
• travel to counties to provide services using agency-provided transportation
• complete plan-of-care goals
• communicate with staff, clients, and supervisors.
