Therapy Care Pharmacist — Acute is a full-time, 100% onsite role in Tempe, AZ, with a Monday–Friday schedule of either 8:30 a.m.–5:30 p.m. or 9 a.m.–6 p.m. Compensation is not specified. The pharmacist supports Soleo Health’s Acute Therapy Care Management Team; Soleo provides specialty pharmacy and infusion services in homes and alternate care settings. The pharmacist assesses clinical information, coordinates treatment plans with prescribers, dispenses specialty medications and supplies, and documents care. The role also supervises support personnel and helps ensure pharmacy services meet clinical, regulatory, accreditation, and company requirements.

What you'll do

• Collect and assess clinical data

• coordinate with pharmacy, nursing, intake, reimbursement, and sales teams
• document patient interactions
• dispense specialty medications and supplies
