Client Care Coordinator

What you'll do

• Coordinate admissions, caregiver matching, and in-person first-shift introductions
• check in with clients and caregivers, resolve concerns, and follow through on solutions
• make targeted quality visits and group nearby stops
• support caregiver orientation and coaching with the RN
• present caregiver recognition awards
• keep WellSky records current
• coordinate with the Branch Manager, RN, and scheduler.

Why this role
