What you'll do

• Teach daily living skills such as cooking, laundry, housecleaning, and personal hygiene
• provide assistance with eating, mobility, transfers, and grooming as needed
• administer or monitor medication
• plan and prepare meals for dietary needs
• support community participation and transportation
• document care and progress
• implement individual support plans
• communicate with families, case managers, and medical professionals.
