What you'll do

• Provide primary care to Medicare patients in a value-based care model
• see approximately 13–16 patients daily
• conduct regular, diagnostic, and new-patient visits
• work with the clinic care team and use available onsite labs and specialist access
• support preventive care and quality outcomes.
