• Assess referred patients for hospice eligibility
• coordinate referrals and admissions with physicians, case managers, and hospital discharge planners
• educate patients and families about hospice philosophy, services, and expectations
• document assessments and coordination accurately and promptly
• assist with admission documentation
• serve as a clinical resource to marketing and business development
• attend interdisciplinary meetings and participate in community outreach as needed.
