What you'll do

• Conduct community-based assessments and crisis interventions at homes, schools, shelters, and other locations
• provide brief counseling, case management, resource linkage, and referrals
• collaborate with internal teams and external partners
• assess and mitigate risk
• connect clients with mental health and substance use treatment and natural supports
• document services in the electronic health record
• participate in case consultation and supervision.
