What you'll do

• Partner with outreach, enrollment, and interdisciplinary team members
• conduct nursing, home safety, and eligibility assessments
• complete Nursing Facility Level of Care assessments and documentation
• present clinical findings at eligibility meetings
• educate prospective participants and caregivers
• maintain accurate records in electronic health systems
• support outreach and community engagement as needed
• contribute to improvements in participant experience and workflows.

Why this role
