What you'll do
- Assess, evaluate, facilitate, and monitor progress toward established goals
- complete intake packets
- provide referrals and crisis intervention
- interview individuals, review records, and confer with professionals about progress and program suitability
- apply knowledge of DSM-5 and ASAM criteria
- collaborate with internal and external partners
- identify and refer individuals with behavioral health needs
- complete program reports and maintain documentation, service plans, progress notes, discharge summaries, and service records
- monitor client progress and database enrollment.
