Residential Service Assistant

Job overview

The Residential Service Assistant role is onsite in Georgetown, DE, with 40 hours per week in 8-hour shifts; schedules may be 8 a.m.–4 p.m., 4 p.m.–midnight, or midnight–8 a.m., based on program needs. Horizon House provides community-based support to adults with developmental disabilities, behavioral health issues, substance use needs, and homelessness. The assistant supports residents’ skill development and independence in a group-home setting. The role documents services and coordinates with treatment-team members, instructors, medical professionals, and family members to support continuity of care.

What you'll do

• Reinforce skills in food management, personal hygiene, relapse prevention, medication management, money management, and planned community activities
• complete progress notes and service plans as required
• maintain dialogue with treatment-team members, vocational and educational instructors, consultants, medical professionals, and family members
• support program service delivery
• respond to emergencies, including inclement weather, as required.
