Care Liaison

The role serves as a primary contact for incoming calls and prescription referrals, helping ensure referrals are entered, processed, and routed appropriately. It contributes to timely responses for patients, caregivers, prescribers, and clients by following program procedures and coordinating prescription delivery.

What you'll do

• Enter prescription referral data
• process and triage referrals
• provide phone support and route calls
• respond to program inquiries
• interact with prescribers
• follow up with clients
• coordinate prescription delivery with patients and caregivers
• share process issues with the operations team
• work cross-functionally with Case Management and Pharmacy triage support.
