What you'll do

• Provide information about hospice services
• complete admission visits and nursing assessments
• follow up with patients awaiting hospice services to support hospital discharge
• document in hospital and hospice records
• gather patient and caregiver information
• communicate assessments and care plans to physicians and care teams
• coordinate discharge planning and hospice enrollment
• provide nursing interventions and symptom management as prescribed.
