Pharmacist — Long-Term Care

The Pharmacist — Long-Term Care position is hybrid, serving healthcare facilities in Mercer and Monmouth counties, NJ. Compensation is $50–$52 per hour. Pharma-Care provides patient-focused consultant pharmacy services throughout New Jersey. The consultant pharmacist evaluates medication regimens and works with physicians, nursing staff, and other healthcare providers to support safe, appropriate medication use. The role also contributes to staff education, medication safety, and quality initiatives across long-term care settings.

What you'll do

• Travel to regional long-term healthcare facilities
• evaluate medication regimen appropriateness and effectiveness
• collaborate with physicians and healthcare providers
• contribute to protocols, antimicrobial stewardship, and drug utilization
• educate nursing staff and healthcare teams on formulary medications, policies, medication safety, and drug shortages
• participate in QAPI and related meetings.
