What you'll do

• Complete initial and ongoing client assessments, including OASIS
• develop and revise plans of care
• evaluate nursing needs and care outcomes
• coordinate with clients, families, physicians, and team members
• document care
• follow infection-control precautions
• supervise, teach, and provide clinical direction to nursing personnel
• perform on-call responsibilities as assigned.
