What you'll do

• Coordinate and oversee hospice patient care across the interdisciplinary team
• Serve as the central point of communication between physicians, caregivers, patients, and internal teams
• Manage physician orders and ensure timely entry and approval
• Partner with Intake to support timely admissions
• Continuously assess patient needs and ensure appropriate reassessment
• Lead development, implementation, and updates of individualized hospice plans of care
• Support patients and families through education, symptom management, and end-of-life care planning
• Review clinical reports and take appropriate action
• Participate in and contribute to IDG meetings
• Perform in-home visits as needed
• Ensure compliance with hospice regulations and documentation standards.
