What you'll do

• Provide primary and preventive care to adults 65 and older, with a focus on chronic disease and prevention, in a value-based care model. Work with an integrated team that includes NPs/PAs, medical assistants, social workers, lab and pharmacy support, and referral coordination. Complete onboarding that includes value-based care, documentation, EMR optimization, and integrated care workflows. Board certification or eligibility in Internal Medicine or Family Medicine is required
• geriatric experience and HEDIS experience are preferred, and value-based care experience is preferred but not required.
