The role supports patients and families through assessment, treatment, education, documentation, and communication with the care team.

What you'll do

• Deliver one-on-one nursing care in patients’ homes
• complete assessments
• follow and contribute to individualized care plans
• administer ordered medications and treatments
• monitor patient condition and document progress
• educate patients and families on care and safety
• communicate condition changes to the care team
• maintain timely clinical documentation.
