What you'll do

• Review clinical referrals and respond to inquiries
• collaborate on hospice eligibility
• assess patients and families
• coordinate assigned caseloads and provide direct care
• provide nursing support, education, and emotional support
• coordinate with hospice physicians
• participate in assigned on-call and holiday rotations
• supervise LVNs and home health aides
• train as backup for hospice admissions.
