Primary Care Physician — Senior-Focused Value-Based Care

What you'll do

• Provide outpatient primary care to senior patients

• perform office procedures including suturing, joint injections, skin biopsies, and basic wound care
• work with a dedicated care team
• support patient satisfaction, HEDIS compliance, and reduced hospital sick days.
