What you'll do

• Accept assigned participants at admission
• complete documentation and orient participants to facility routines
• learn relevant personal and social information
• observe and respond to changes in mental and physical health
• monitor and support participants’ personal and treatment goals
• counsel participants on problems, values, responsibilities, and relationships
• assist with personal futures planning
• provide local transportation as required.
