What you'll do

• Complete patient databases, treatment plans, progress notes, and discharge summaries
• orient patients to program objectives, rules, and rights
• assess and place patients according to treatment phases
• counsel patients in crisis or regarding program requirements
• arrange referrals when appropriate
• review treatment plans
• participate in case conferences
• maintain patient files and submit reports.
