What you'll do

• Conduct field-based outreach and assess eligibility
• complete comprehensive assessments and individualized care plans
• coordinate care and navigate healthcare, behavioral-health, social-service, and community resources
• accompany members to appointments when necessary
• assist with benefits and SSI application advocacy
• identify housing resources
• document, report, and analyze data
• maintain an average caseload of 27 members, subject to change.

Why this role
