What you'll do

• Under direct supervision, assess patients’ behavioral health needs
• provide evidence-based individual, family, and group treatment
• document clinical interventions and diagnoses in the electronic medical record
• develop treatment plans and measurable goals
• coordinate care with patients’ care teams
• participate in weekly referral reviews and quarterly peer review
• and provide wellness education and referrals.
