What you'll do

• Provide skilled nursing care under physician treatment plans
• assess patients, families, and home situations
• document care and complete OASIS assessments
• coordinate with physicians and other disciplines
• supervise LPNs and paraprofessionals
• support patient and family education
• and complete weekend-option reporting and paperwork.
