The Senior Product Manager, Front-End Applications role is remote, with regular travel for onsite go-lives, department rounding, and other support activities as needed. The stated base salary range is $170,000–$194,000 per year in San Francisco, New York City, or Seattle, and $149,000–$185,000 per year elsewhere; the role is also eligible for a target annual bonus of 10% of base salary. Habitat Health provides integrated medical and social care through its PACE model for older adults. The role exists to own the strategy, roadmap, delivery, adoption, and iteration of applications supporting clinical, operational, growth, and administrative workflows. It translates stakeholder needs into product priorities and coordinates internal teams and vendors to improve systems as Habitat Health expands its PACE centers and operations.

What you'll do

• Own product vision, strategy, and roadmap for applications including call and communications platforms, CRM, and EHR. Translate stakeholder needs into prioritized backlogs and roadmaps
• define and track adoption, utilization, satisfaction, and operational-impact metrics. Lead discovery, requirements, user stories, backlog grooming, sprint and release planning, and application implementations for new PACE centers. Manage vendor relationships and support solution evaluation and selection. Coordinate rollout, onboarding, training, change management, documentation, and user-feedback processes.
