Service Coordinator

What you'll do

• Provide service coordination under New York State Department of Health NHTD and TBI waiver services
• conduct home visits and community education
• develop, review, and submit service plans
• maintain detailed records and time tracking
• participate in program development
• and provide case management for up to 25 consumers.
