What you'll do

• Follow each resident’s individualized service plan
• observe and report changes in condition and document care, including night rounds
• check and record vital signs and weights as assigned
• assist with bathing, oral care, toileting, dressing, positioning, and mobility
• provide behavioral prompts and support
• assist with activities and transfers
• respond to urgent resident calls
• maintain clean and safe areas
• support fall prevention and a restraint-free environment.
