The role exists to build and maintain referral relationships, perform clinical screenings to determine admission appropriateness, and communicate rehabilitation options to patients and families. The Clinical Liaison serves as the primary point of contact for external case managers and discharge planners to ensure seamless patient transitions into the rehabilitation program.
