The role coordinates psychosocial support and care access for people living with HIV, helping address medical, social, and everyday needs. Through assessment, advocacy, referrals, and collaboration with patients, families, providers, and community partners, the position supports continuity of care and improved health outcomes.

What you'll do

• Screen referred individuals for social service needs
• assist with program eligibility and drug assistance applications
• enter and maintain patient information in CPCDMS
• assess medical and psychosocial needs
• provide referrals and linkage to services
• support medication and visit adherence
• advocate to reduce barriers
• document services in the EHRS record within 72 hours
• participate in quality assurance and performance improvement.
