What you'll do

• Collaborate with nurse practitioners using internal care delivery technology and clinical judgment to provide virtual care
• help develop and refine clinical programs addressing patient needs and care gaps
• work with physicians, nurse practitioners, care managers, and other care-team members to support care coordination and high-quality care
• provide supervision for nurse practitioners as part of the interdisciplinary care model.

Why this role
