What you'll do

• Assess patients’ and families’ psychosocial status and communicate findings to the interdisciplinary group
• conduct social evaluations and provide counseling or crisis intervention as needed
• educate patients and families about advance directives
• participate in individualized care planning and team meetings
• assist with funeral, financial, legal, and healthcare decision planning
• maintain clinical notes
• coordinate community referrals and continuity of care
• participate in on-call duties.
