Job overview

What you'll do
- Conduct initial and ongoing patient assessments
- develop and update care plans per physician orders
- administer medications, IV therapy, wound and catheter care, and other skilled interventions
- monitor patient progress
- educate patients and families
- coordinate with physicians, therapists, and the home care team
- document care
- guide aides and CNAs as needed
- participate in OASIS assessments, care conferences, and discharge planning
- comply with HIPAA, infection control, and Massachusetts regulations.

Why this role
