Family Residential Program Case Manager

What you'll do

• Conduct intake and needs assessments and develop service plans with residents
• maintain regular contact and monitor progress
• help address barriers and connect families with community resources
• coordinate with community partners
• support admissions, orientation, and discharge
• maintain case records and prepare reports
• facilitate or assist with groups and workshops
• provide crisis intervention and follow agency safety and reporting protocols
• collaborate with agency staff.
