What you'll do
- Provide clinical therapy management
- collaborate with providers on medication reviews and ongoing management
- facilitate monthly polypharmacy meetings
- participate in the Senior TLC Falls Committee and co-facilitate the Pharmacy Committee
- monitor drug utilization and pricing data
- lead the Part D Risk Assessment process with quality and compliance leaders
- participate in care planning and document goals and interventions.
