Case Manager — Transitional Housing Program

The Case Manager — Transitional Housing Program works onsite in Vista, CA, for $21–$30 per hour. The role supports North County Lifeline’s transitional housing services for foster and probation youth moving toward independent living. The case manager provides intensive, client-centered support to help youth build life skills, pursue education and employment, and develop plans for independence. Coordination with community providers and crisis support contribute to participants’ safety, well-being, and long-term self-sufficiency.

What you'll do

• Build rapport and identify participants’ strengths and barriers
• provide intensive case management and life-skills development
• support education, employment, housing, and legal-service goals
• coordinate referrals and care
• provide crisis intervention and safety planning
• develop and monitor individualized case plans
• conduct outreach
• coordinate with housing specialists and community partners
• maintain timely client documentation.
