Registered Nurse (RN) Case Manager

What you'll do

• Coordinate care for patients with chronic, acute, or complex conditions
• conduct comprehensive physical, psychosocial, and environmental assessments
• develop, implement, and evaluate individualized care plans with interdisciplinary teams
• monitor progress and adjust plans
• advocate for patient needs and preferences
• coordinate communication, appointments, referrals, transportation, and community resources
• support appropriate resource utilization
• educate patients and families about diagnoses, treatment options, and care planning.
