Senior Intake Coordinator

The Senior Intake Coordinator role is onsite in Canton, MA for training; work from home may be available after training. Pay is $23–$24 per hour. New England Life Care is a nonprofit home infusion provider serving hospital systems across New England. The coordinator manages comprehensive intake coordination for patients receiving home infusion therapy. By compiling and maintaining patient information and communicating referral, care, and delivery needs across clinical and operational teams, the role helps support coordinated patient care and efficient discharge.

What you'll do

• Compile initial patient profiles and distribute them to the healthcare team
• answer and document intake calls
• communicate referrals and patient issues with pharmacy, reimbursement, materials management, delivery, nursing, and care coordination staff
• document patient information in clinical records
• communicate changes in supply requirements
• process requested deliveries
• update patient-file errors and referral information.
