The Registered Nurse (RN) – Home Health role is an onsite, full-time or part-time position based in Elyria, OH, providing care in clients’ homes. Compensation is based on $45 per point, with $50 per day for on-call rotations and $0.43 per eligible mile; mileage is not paid for travel to the first or from the last visit of the workday. Always Best Care Grafton supports clients’ safety, comfort, and independence at home. The RN provides skilled nursing care and clinical oversight according to individualized care plans. Assessments, care coordination, treatment, education, and accurate documentation help clients and families receive appropriate home-health services.

What you'll do

• Provide skilled nursing care under individualized care plans
• complete nursing and OASIS assessments and reassessments
• monitor health changes
• develop and evaluate care plans
• coordinate with physicians, therapists, caregivers, clients, and families
• provide education
• perform wound care and other procedures within scope
• administer ordered medications and treatments
• maintain clinical documentation.
