Case Manager

The Case Manager role is onsite in New York, NY. Pay is $50,000 per year. The role coordinates counseling, case management, and aftercare, connecting clients with support services and helping them build plans and capacity for long-term self-sufficiency.

What you'll do

• Conduct intake and psychosocial assessments
• orient clients to the shelter program
• develop Independent Living Plans with social service providers
• connect clients with public assistance and community resources
• monitor progress and hold regular client meetings
• maintain confidential case records
• report critical incidents
• coordinate with housing and employment staff
• provide periodic case management and counseling for up to one year after client departure.
